If you have received the correct meningitis vaccine within 5 years of starting classes, you can submit a photo/copy of your shot record below.

You are exempt from the meningitis requirement if:

• You are 22 or older or

• You have been continuously enrolled at San Jac

​

• Distance Learning waiver (4th box)

• TX waiver

• Temp waiver due to a medical condition (5th box and must come with letter from a physician)
